Sports Physical Exams
If
you're planning to participate in athletics and you still haven't gotten your
required physical exam, you can click
on this link to open up the form. Print the form, complete the medical
history questions on the second page, then bring the form with you to your exam.
Remember, if you want to participate in athletics at St. Therese,
you cannot do so without a physical!
If
you still need to get a physical, you can get one at Holy Ghost School (San
Pedro just north of Gibson) on Saturday, August 23 from 8:00AM until 12:00 Noon.
Cost is $20.00.
Mandatory
Sports Parents Meeting
All parents for Cheerleading, Volleyball and
Football need to attend (mandatory for child's participation) the Parents
meeting. We had one 8/11, but there are some that still need it. We will have a
conference call on Sunday August 24 at 1:00 to meet the requirement. One of the
parent or guardians needs to be on the call if you missed the first meeting. It
is easy and you can do it from anywhere: call 1-800-391-1709 at the prompt enter
the conference code # 595690. You will be on and can ask questions. It will be a
15-20 minute call and we will take roll at the end for credit on the call.
For those parents participating in the August 24
